A, Gapsalskaya St., Municipal Okrug Morskie Vorota, Saint Petersburg, 198035, Russia, as the administrator of the website https://dpnit.ru and the Nit: Mental Health mobile application, to process the personal data I have provided, including my surname, given name, patronymic, passport data, date of birth, place of birth, phone number, registration address, email address, INN, cases of contacting specialists, data transmitted as part of files, education, specialization, and any other information I provide, as well as information automatically received when accessing the Website: IP address, operating system and browser information, device type (desktop computer, mobile device, etc.), geographic location, and cookie data (subject to the user’s browser settings).
This consent is granted for any action or set of actions performed with or without automation tools in relation to personal data, including collection, recording, systematization, accumulation, storage, clarification (updating, modification), extraction, use, transfer (dissemination, provision, access), depersonalization, blocking, deletion, and destruction of personal data, in accordance with Articles 3, 9, and 15 of the Law, for the purpose of providing services (performance of agreements and contracts with the user), identifying the user as a party under such agreements and contracts, contacting the user, improving quality and developing new services, and for other purposes determined by the Operator’s chartered activities.

I also agree that completing and submitting forms, registering on the Operator’s website https://dpnit.ru or in the Nit: Mental Health mobile application, after prior review of this text, constitutes, in accordance with Article 9, Clause 1 of the Law, sufficient consent to process my personal data. This form of consent confirms the fact that my consent has been obtained, and no written form or other evidence is required for additional confirmation of my free will by the Operator. My consent is specific, informed, and conscious.
